PROVIDER QUALIFICATIONS


Physical Therapy Services Must Be Provided By:

• A Physical Therapist Licensed by the Virginia Board of Medicine

• A Graduate (trainee) of a physical therapy program, approved by the American Medical Association and American Physical Therapy Association who is engaged in clinical experience under the direct supervision of a licensed physical therapist as described above

• A physical therapist assistant licensed by the Virginia Board of Medicine and directly supervised by a licensed physical therapist as described above


Occupational Therapy Services Must Be Provided By:

• An occupational therapist licensed by the Virginia Board of Medicine

• A graduate of an occupational therapy program approved by the American Medical Association or the American Occupational Therapy Association who is engaged in clinical experience under the direct supervision of a licensed occupational therapist as described above

• An occupational therapist assistant certified by the National Board for Certification in Occupational Therapy and is directly supervised by a licensed occupational therapist as described above


Speech-Language Pathology Services Must Be Provided By:

• A Masters level prepared SLP licensed by the Va. Dept. of Health Professionals, Board of Audiology and Speech-language Pathology

• A SLP with their Certificate of Clinical Competence (CCC) from ASHA

• A SLP who is licensed by the Board of Education with an endorsement in Speech-Language Disorders, prek-12 and a masters degree, licensed without exam by the Board of Audiology and Speech-Language Pathology

• A SLP licensed by the Board of Education with an endorsement in Speech Language Disorders, prek-12, but does not hold a master’s degree in Speech-Language Pathology who must be directly supervised by a Board of Audiology and Speech Language Pathology or a SLP with their CCC


Skilled Nursing Services Must Be Provided By:

• A Registered Nurse or Licensed Practical Nurse licensed by the Virginia Department of Health Professionals

• A Licensed Practical Nurse must be supervised by a Registered Nurse as required by the Code of Virginia


Hearing Screens Must Be Provided By:

• A Nurse Practitioner, Registered Nurse, Licensed Practical Nurse, Licensed Audiologist,or a Speech-Language Pathologist


EPSDT

• Must Be Provided by a Physician or a Nurse Practitioner

• This is an unclothed Physical Examination that can be completed according to a periodicity schedule.

• Only the EPSDT can be billed to Medicaid/CMSIP, no episodic care.

• Copies of all physicals should be sent to recipients primary care physician.
