Sample evaluation prototypes for teachers, administrators, and superintendents have been developed. The prototypes combine the criteria suggested in the Guidelines for Uniform Performance Standards and Evaluation Criteria for Teachers, Administrators, and Superintendents with best practices, research and stakeholder input to guide school division personnel in designing performance evaluation tools. These documents provide suggestions and samples that school divisions may (1) use in total; (2) revise to suit local needs and preferences, or (3) use as a reference.

Each division superintendent and teacher approved program dean or chair will receive one notebook containing the full document, Sample Evaluation Prototypes for Teachers, Administrators, and Superintendents. Virginia educators may duplicate these materials. The sample prototypes may be accessed below:
