The Self-Assessment includes six questions addressing how:

1. the school division reviews data to identify problems/concerns,
2. the data review is used to identify root causes that will improve the understanding of the factors that contribute to the problems and concerns,
3. information and documentation are used when making decisions,
4. professional development activities are planned and implemented,
5. the school division plans for improving efforts and sustaining quality activities, and
6. action plans are designed.
