  We need your assistance with the collection of the 1995-96
  Status Questionnaire data and the completion of the 1996-97
  Driver Education Program Approval Application.  This
  information will also be mailed directly to your principals,
  and should be received by the Department no later than June
  14, 1996.  
  
  The Status Questionnaire information will determine the
  amount of state reimbursement your school division will
  receive for in-car driver education instruction.  The
  additional program information included on the form will be
  compiled into a state-wide report which will be available
  upon request.
  
  The Driver Education Program Approval Application must be
  completed by the individual schools planning to conduct an
  in-car and/or classroom program during the 1996-97 school
  year.  Any program or staff changes reported on the form
  will be communicated to the Department of Motor Vehicles.   
  In addition,  please also provide the place and date of the
  paraprofessional driver education in-service training as
  required by the Code of Virginia.
