  Enclosed is the form to be used for the annual report for
  Homebound Instruction provided during the 1995-96 school
  year.  State funding for this program is based on prior year
  data, and the information reported will be used to determine
  the 1996-97 allocation for this program.
  
  NOTE:  Please complete a separate reimbursement form for
  each different hourly rate paid to the homebound teachers
  employed by your school division, along with the requested
  information for each teacher.
  
  Do Not claim reimbursement on this form for services to
  properly long-term suspended or expelled students with
  disabilities covered under the Department's Continued
  Service Provision process.
  
  The completed form(s) should be returned on or before July
  15, 1996 to:
