  Enclosed is the form to be used for the annual report for
  Homebound Instruction provided during the 1997-98 school year. 
  State funding for this program is based on prior year data,
  and the information reported will be used to determine the
  1998-99 allocation for this program.  

  NOTE:  Please complete a separate reimbursement form for each
  different hourly rate paid to the homebound teachers employed
  by your school division, along with the requested information
  for each teacher.

  The completed form(s) should be returned on or before July 15,
  1998 to:

  In the event that "NO" State funds are being requested for
  this funding category for the 1997-98 school year, please mark
