| SUBJECT: | School Health Advisory Board Annual Report |
Please find attached a copy of the 1999 School Health Advisory Board (SHAB) Annual Report Form. The Code of Virginia (22.1-275.1), requires each school division to have a School Health Advisory Board comprised of no more than 20 community representatives. In addition, each School Health Advisory Board is required to submit an annual report on the status and needs of student health in the school division to any relevant school, the school board, the Virginia Department of Health, and the Virginia Department of Education. The SHAB chairperson or contact person is required to submit two copies of the Annual Report Form to the Department of Education by September 1, 1999 to: Fran Anthony Meyer, Specialist Department of Education Office of Special Education and Student Services P. O. Box 2120 Richmond, VA 23218-2120. The Department of Education will forward one of the copies to the Virginia Department of Health. Questions about the SHABs or the Annual Report Form may be directed to Dr. Meyer at (804) 225-4543, fmeyer@pen.k12.va.us, or to Mrs. Gwen P. Smith, R.N., M.S.N. at (804) 786-8671, gpsmith@pen.k12.va.us. Also included is a form SHAB chairs/contact persons are asked to complete. This information is needed to update the SHAB Directory. The person listed in the directory serves as the point of contact for mailing resources or other information to each SHAB throughout the year. Copies of previous years? summary reports of local School Health Advisory Boards' Annual Reports are available upon request. If you would like a copy, please contact Dr. Meyer or Mrs. Smith.
