SUBJECT: 2000-2001 Status Questionnaire and 2001-2002 Driver Education Program Approval Application

We need your assistance with the collection of the 2000-2001 Driver Education Status Questionnaire data (see Attachment A). The information contained in this questionnaire will be used to determine your 2000-2001 per pupil basic aid reimbursement for in-car instruction. We would appreciate receiving this information no later than July 2, 2001.

Also, an application (see Attachment B) for driver education program approval is attached. The application must be submitted prior to offering driver education instruction during the school year 2001-2002. The classroom and in-car teacher information contained in the application will be shared with the Department of Motor Vehicles to satisfy the Code of Virginia driver record monitoring requirement. Students who receive instruction from a teacher without proper endorsement are not eligible for a Virginia provisional driverís license.
