SUBJECT:

U.S. Department of Educations Consolidated State Performance Report: Request for Certain Federal Program Data for School Year 2002-2003

 

The U.S. Department of Education has released the second phase of its 2002-2003 Consolidated State Performance Report due June 30, 2004. This phase requests certain information that is not available through the Virginia Department of Educations Information Technology (IT) office. We ask that you forward the Excel file survey in Attachment A to the appropriate federal program directors in your division. We also ask that one director assume responsibility for completing and returning the survey. Typically, the Title I coordinator assumes this role. The programs included in the survey are:

 

Title I, Part A Local Education Agency's Basic

Programs

Title V, Part A Innovative Programs

 

Return one completed survey by e-mail to consperf@mail.vak12ed.edu by June 1, 2004. Also return a signed print copy of the data certification form to Mrs. Delores (Dee) Audet, Virginia Department of Education, Office of Program Administration and Accountability, P.O.
