SUBJECT:	2004-2005 Driver Education Status Questionnaire

The driver education laboratory information submitted on the Web-based Status Questionnaire will be used to ensure program compliance in accordance with 22.1-205 of the Code of Virginia and to identify school divisions that are not eligible to receive Driver Education Funds in school year 2005-2006.

Please complete the attached 2004-2005 Status Questionnaire form. We would appreciate receiving this information no later than June 30, 2005.
