Student Data Collection for Homeless Children and Youth
The purpose of the memorandum is to finalize the request for school division data related to homeless children and youth during the 2004-05 school year that was initially announced in Superintendents Memo 239, dated December 3, 2004. This information is required by the U. S. Department of Education and is similar in format to the data request for 2003-04.
School divisions that are not subgrantees receiving direct McKinney-Vento funding under Project HOPE-Virginia should complete the three-page form. Note that shaded boxes reflect information already being reported to the Virginia Department of Education. This information need not be duplicated and will be accessed from your student record collection submission. Subgrantees must submit the more detailed form as part of their annual report.
The finalized report forms (based on additional changes received in April 2005) are attached. A copy of the appropriate form and a technical assistance guide is being sent directly to the local homeless education liaison in each school division.
Please complete the appropriate form for your school division. The completed form may be mailed, faxed, or e-mailed to Project HOPE-VA.
In order to adhere to the U. S. Department of Education submission date for states, please submit your completed form no later than September 1, 2005, to Project HOPE-VA. If you have any questions, please contact:
