Annual Report
for Homebound Instruction 2004-2005 |
In the past, Homebound Instruction Services data have been collected through a paper form, which included the teachers name, social security number, number of hours of instruction and the hourly reimbursement rate. In order to simplify the reporting of this information, a new Web-based system has been developed. The Single Sign-On for Web Systems (SSWS) allows each user to have access to many different data collection processes, as well as other applications, with one single identification and password.
The data must be submitted electronically through
the SSWS, which may be accessed at: https://eb01.vak12ed.edu/ssws.
Individuals who will be
entering these data will need to contact the designated Single
Sign-On for Web Systems (SSWS) Account Manager designated for their school
division to obtain a password.
Instructions explaining the new automated Annual Report for Homebound
Instruction reporting system are in Attachment A.
The completed form must be printed, signed by your
local superintendent and returned by September 15, 2005 to Gwen P. Smith,
school health specialist, Office of Student Services, Department of Education,
P.O. Box 2120 20th floor, Richmond, VA 23218-2120.
