SUBJECT: |
Annual
Report for Homebound Instruction 2005-2006 |
The Board of Education through the Regulations Establishing Standards for Accrediting Public Schools in Virginia requires that homebound instruction shall be made available to public school students confined at home or in a health care facility for periods that would prevent normal school attendance. Fiscal year 2007 funding is provided through the current appropriations act.
The
reporting of Homebound Instruction Services data for school year 2005-2006 will
be collected through the Single Sign-On for Web Systems (SSWS) that allows each
user to have access to many different data collection processes, as well as
other applications, with one single identification and password. Instructions explaining the automated Annual
Report for Homebound Instruction reporting system are in
Attachment A. The data requested will
include: homebound teachers license number, hourly rate of payment and number
of hours worked.
The data must be submitted electronically through
the SSWS, which may be accessed at: https://eb01.vak12ed.edu/ssws.
Individuals who will be
entering data and do not have a current homebound password will need to contact
the designated Single Sign-On for Web Systems (SSWS) Account
Manager designated for their respective school division.
The completed form must
be printed, signed by the superintendent, and mailed by October 1, 2006, to
Gwen P. Smith, school health specialist, Office of Student Services, Department
of Education,
