Section 22.1-212.11. of the Code of Virginia requires local school boards to report annually to the Board of Education the number of public charter school applications that were granted or denied, the reasons for such denials, and whether the charter was designed to increase the educational opportunities of at-risk students. After the information is collected, the Board of Education must report the findings to the Governor and the General Assembly.

The form can be returned by fax to Diane Jay, associate director, Office of Program Administration and Accountability, at (804) 371-7347, or sent by e-mail with an electronic signature, to Ms. Jay at Diane.Jay@doe.virginia.gov.
