  Please find attached a copy of the 1996 School Health
  Advisory Board Annual Report Form.  According to the Code of
  Virginia (22.1-275.1) each school division is required to
  have a School Health Advisory Board (SHAB) comprised of no
  more than twenty (20) community representatives.  In
  addition, each School Health Advisory Board is required to
  submit an annual report on the status and needs of student
  health in the school division to any relevant school, the
  school boards, the Virginia Department of Health, and the
  Virginia Department of Education.
  
  In reporting to the state agencies, the School Health
  Advisory Board Chairperson should submit two (2) copies of
  the Annual Report Form to the Department of Education by
  July 1, 1996.  The Department of Education will forward one
  of the copies to the Virginia Department of Health.

  An analysis of the 1995 and 1996  SHAB Reports is near
  completion.  Copies of the analysis of the 1994 School
  Health Advisory Boards' Annual Reports will be available
