  I. NUMBERS of Identified Gifted Students:

     Complete this chart by providing the numbers of students
     identified by category and grade level as indicated on the form.

     In the space under the chart, please indicate the total number of
     identified students served in your division.  PLEASE REMEMBER TO 
     COUNT EACH STUDENT ONLY ONCE.

  II.ETHNIC GROUP Breakdown:

     Provide data for school division population by ethnic categories 
     as indicated.  In the second column provide numbers of students in
     each group identified as gifted.
