SUBJECT: School Health Advisory Board Annual Report

  Please find attached a copy of the 1997 School Health
  Advisory Board (SHAB) Annual Report Form.  According to the
  Code of Virginia (22.1-275.1) each school division is
  required to have a School Health Advisory Board comprised of
  no more than twenty (20) community representatives.  In
  addition, each School Health Advisory Board is required to
  submit an annual report on the status and needs of student
  health in the school division to any relevant school, the
  school boards, the Virginia Department of Health, and the
  Virginia Department of Education.

  Due to a later distribution date of the 1997 Report Form,
  the deadline for submitting the annual report to the state
  agencies is extended.  The SHAB Chairperson should submit
  two (2) copies of the Annual Report Form to the Department
  of Education by September 1, 1997.  The Department of
  Education will forward one of the copies to the Virginia
  Department of Health.

  Attached also is a form SHAB Chairs are asked to complete
  for updating the SHAB Directory.  The local person listed in
  the directory serves as the point of contact for mailing
  resources or other information to each SHAB.  

  Copies of previous years' summary reports of the local
  School Health Advisory Boards' Annual Reports are available
  upon request.  If you would like a copy, please contact Fran
  Meyer at the Department of Education.

               will be sent to the superintendent's office
