SUBJECT: School Facility Survey

  We have been asked by The Joint Committee to Study
  Educational Infrastructure (HJR 165, 1998) to mail out and
  receive your completed responses of the enclosed survey. 
  Please notice that Section 1: School Division Data (pages
  2 through 6) needs to be completed once for your division
  by an individual that has overall knowledge of your budget
  and spending patterns.  

  Section 2: Elementary School Data (pages 1 through 11),
  Section 3: Middle School Data (pages 1 through 11), and
  Section 4: High School Data (pages 1 through 11) will need
  to be duplicated and distributed to each of your schools
  and centers for completion by someone who is very familiar
  with the particular facility.  Please choose the section of
  the survey that most closely describes the grade pattern in
  each facility.  

  A combined school may need to use two or three sections but
  you only need to answer the duplicated questions on
  building condition one time.  If multiple sections are
  used, please staple the survey together and note on the
  first page the special conditions at that school.

  For consistency and accuracy, we ask that you coordinate
  the responses and any questions through your central
  office.  Since the Department of Education did not develop
  the survey, we cannot interpret or explain the intent or
  meaning of individual questions.  Please do your best to
  answer all questions completely to the best of your

  Facilities Services will do their best to answer any
  questions that may develop, but we do ask that you try to
  answer the questions from individual schools first before
  calling Richmond.  If the questions cannot be answered at
  your division, please call David Boddy, Hunter Barnes, or
  Vijay Ramnarain at  (804) 225-2035 and indicate you have a
  survey question.

  Please package all of your division's responses together at
  your central office prior to November 30, 1998, and mail

  A cover letter included with your submission indicating the
  surveys enclosed and any special circumstances or missing
  schools would be a big help in tabulating the responses.
