The attached EB.026 Form, "Requisition for Supplies-Virginia School Bus Safe Driving Awards Program," should be submitted when ordering school bus safe driving award items. There is no charge for the basic safe driving award certificates and cards; however, there is a charge for the numbered year panels, patches, lapel pins, and key chains. Second year recipients will receive a certificate, card, numbered year panel, and a patch. All other recipients will receive a certificate, card, and appropriate numbered year panel. All requirements for participation in the Virginia School Bus Safe Driving Awards Program by the school division and eligibility for drivers, as set forth in the program guide, must be met prior to the presentation of these awards. The awards program is now in its nineteenth year. Only first through nineteenth year awards or additional shoulder patches can be ordered for the 1998-99 school year. Please do not order additional number panels or certificates as backup stock. Orders will be shipped and invoiced as soon as possible after receipt of the requisition form. Please allow two weeks for delivery. Most deliveries will be made by United Parcel Service (UPS); therefore, please make certain that the mailing label is completed on the requisition form for UPS delivery. UPS will not deliver to a post office box. Upon receipt of the award items, please open and inventory immediately.
