EQUIVALENCY) TO PROVIDE SPECIAL EDUCATION AND RELATED SERVICES FOR CHILDREN AND 
YOUTH WITH DISABILITIES and instructions for completing this report.  This report is 
required under the provisions of Part B of the Individuals with Disabilities 
Education Act.

Please return the Teachers Employed Report by Friday, June 30, 2000, to
