SUBJECT:	2003-2004 Driver Education Status Questionnaire

The driver education laboratory information submitted on the Web-based Status Questionnaire will be used to calculate your 2003-2004 per pupil state basic aid reimbursement for in-car instruction and to ensure program compliance in accordance with 22.1-205 of the Code of Virginia.

The 2003-2004 Status Questionnaire form is accessible at http://www.pen.k12.va.us/VDOE/Instruction/PE/statusquestionaire.pdf.
