Annual Report for Homebound Instruction 2003-04

Enclosed is the form to be used for the Annual Report for Homebound Instruction provided during the 2003-04 school year. State funding for this program is based on prior year data, and the information reported will be used to determine the 2004-05 allocation for the program.

NOTE: Please complete a separate reimbursement form for each different hourly rate paid to the homebound teachers employed by your school division with the requested information for each teacher.

The completed form(s) should be returned on or before August 15, 2004, to Gwen P. Smith, school health specialist, Office of Student Services, Department of Education, and P.O. Box 2120, Richmond, VA 23218-2120.

In the event that no state funds are being requested for this funding category for the 2003-04 school year, please mark the form zero (0), sign, and return as indicated above.
